Category B:
1. 4 copies of O’ Level Result(s), (issued by examination body)
2. 4 copies of the Atiba Provisional Admission letter
3. 4 copies of the JAMB admission letter- Note that admission will be treated Two weeks after
resumption.
4. 4 copies of Court Affidavit form of non-membership of any cult or secret society
5. 4 copies of Birth Certificate
6. 4 copies of citizenship / state of origin certificate
8. 4 copies of the JAMB change of Institution (where applicable)
9. 12 recent Passport photographs
Recommendation letters from two different people of high standing within the community
10. 4 copies of the receipt of payment for application form
11. 4 copies of A’level/NCE/Degree results used for application by Direct Entry Students (where
applicable)
12. 4 Copies of Atiba application form
13. Error-Related Court Affidavits where applicable (e.g change of name/ change in arrangement
or spellings of name etc.)
14. Four Copies of all receipts of payments from the student portal.
Category C:
Sequel to the series of medical issues the University has had to contend with the students on campus after submitting medical examination from recognized government hospitals. It has become imperative for the University to conduct the following medical examination on all fresh students in order to ascertain the health status of each of the students.
In view of the above, the following medical tests must be carried out on all students at the University
Clinic on resumption, at a fee of N32,500:00 only
1. Genotype
2. Blood group
3. RVS
4. HBV
5. VDRL
6. Urinal analysis
7. Stool Microscopy
8. Report on any special medical needs of the student
